Williams County Health Department can issue birth certificates to anyone born in the state of Ohio. You can make a request by phone, mail, online, or in person at our office. Home births are also registered by the Health Department.
The information we need to complete your request includes:
- Name at birth on the certificate
- Date of birth on the certificate
- Maiden name of the mother
- Father’s name, if applicable
- The city and county where the birth occurred

Mail Orders Application for Birth Certificate
Please complete this the Application for Birth Certificate. Print and attach the appropriate fee ($25 for each certificate). Orders are processed the day they are received.
Or send the following information:
- Birth information: name at birth, date of birth, mother’s maiden name, father’s name (if applicable) and where the birth occurred
2. Check or money order for $25.00 for each certificate requested made out to “Williams County Health Department”
3. A self-addressed, stamped envelope
4. Daytime phone number where you can be reached for questions

Online Orders
Orders can be placed online using the VitalChek website. VitalChek adds a service fee for processing this order and sends it to us to prepare. Expedited shipping is offered. The birth request will go to the county where the person was born. Be advised we cannot provide status of an order of someone not born in Williams County (example: if you were born in Williams County, we will get your request, if you were born in Columbus, Franklin County will get your request).

Visit our main office in Montpelier and your certificate is prepared while you wait. We accept cash, credit/debit card, check or money order for same day service.
